Utilizing Standardized Assessments to Inform the Service Planning Process: Part 2 - SIS (Document Success Series)

Documentation can be tough.  Despite being a key component to service delivery, many providers struggle with teaching and training staff to consistently produce documentation that tracks and demonstrates progress, is measurable, is observable, meets minimum standards for billable notes, is person-centered, adequately notes risk, etc.  This training series will equip providers with tools to aid in great documentation at multiple steps through the process. Starting with assessments and concluding with a web-based documentation training for DSPs, you’ll be sure to gain tools and ideas to enhance your documentation.

April 4, 2019 @ 9 AM - Utilizing Standardized Assessments to Inform the Service Planning Process: Part 1 - InterRAI

Description: The developers of the interRAI assessment instruments have created Clinical Assessment Protocols (CAPs) for the interRAI Community Mental Health (CMH) and Collaborative Actions Plans (CAPs) for the Child and Youth Mental Health (ChYMH) as an evidence-informed approach to using assessment results for comprehensive service planning.  The CAP components include issues, goals of care, triggers, guidelines, and additional resources.  The CAPs for both versions of the interRAI will be reviewed, with an emphasis on identifying specific items in each version of the assessment which trigger CAPs to be considered for an individual during the person-centered planning process. 

April 10 @ 10:30 - Utilizing Standardized Assessments to Inform the Service Planning Process: Part 2 - SIS 

Description: Participants will learn how to use the SIS as one tool in the person-centered planning process. 

April 25 @ 10 AM - Webinar: Writing Goals, Objectives, and Interventions

Description: This training will look at how to write the core pieces of a person's service plan that assists in setting up a strong plan that is clear to the person served and that allows for stronger staff document. Particular focus will be on the importance of identifying barriers that drive the objective and how to write a strong, measurable, and achievable objective. 

April/May Regional Training (4/29/19 - 5/2/19) - Training and Coaching DSPs for Excellence in Documentation

Description:  This training will look at how to educate DSP's on how to write a strong documentation note. It will walk through the role of the plan writer but then focus on the core documentation elements and how to best engage staff in this writing process. Specifically, the training will identify required pieces of documentation along with how to assist DSP's in including all the elements in a clear and concise way.  

June 20 @ 1 PM - Documentation for HCBS Services

Description: This training will educate DSP's on how to write a strong documentation notes. It will walk through the core elements to documentation, as well as cover how to use the already established goals and objectives to enhance the contents of the note.
